What are my options when my teeth fall out due to periodontal disease?

Q. Hi, I have been petrified of dentists for years so have avoided going at all costs, but over the last year i noticed the gap and gum between my top front teeth was getting bigger so i plucked up the courage to go and see my local dentist. Once there she gave me an xray and said I had periodontal disease very severe, since then I have been back for her thoroughly clean them and remove any tartar that has built up but she has now told me that chances are i am going to lose my teeth by the end of the year (they could fall out) as you can understand this has scared me as I am only 32 and she doesnt seem to be giving any form of advise as to what I can do to a) stop it getting any worse or B) if they do fall out what can be done, I understand I am with an NHS dentist but was wondering what options I would have if I went private as i dont really understand what you price list means, if you could give me any advose I would be very grateful.
